1. Overview

Prednisolone is an adrenal cortical hormone drug. The role and use are the same as dexamethasone. It is easily absorbed by the digestive tract, exists in an active form, and exerts its biological effects without conversion by the liver. The plasma concentration of the drug reaches its peak in 1 to 2 hours after oral administration, and the half-life is 2 to 3 hours. In the blood, most of this product is bound to plasma proteins (but the binding rate is lower than that of hydrocortisone), and the free and bound metabolites are excreted in the urine, some are excreted in their original form, and a small part can be excreted in milk.

2.indications

For allergic and autoimmune diseases.

3.clinical applications

Oral: 10~40mg/day for adults, divided into 2~3 times. Those with normal body temperature should take it once a day in the morning. The maintenance dose is 5 to 10 mg/day. Intramuscular injection: 10~30mg/day. Intravenous infusion: 10~20mg/time, dissolved in 500ml of 5% glucose solution. Injection into the joint cavity or soft tissue (suspension), 5-50 mg/time, the dosage depends on the size of the joint.

General surgical patients should try not to use, so as not to affect the wound healing. Both this product and cortisone need to be metabolized by the liver to be activated into prednisone or hydrocortisone to be effective, so patients with poor liver function should not be used. Hypertension, hyperproteinemia, heart disease, acute heart failure, diabetes, psychosis, hypothyroidism, osteoporosis, glaucoma, acute herpes simplex keratitis, cowpox, chickenpox, infectious diseases, most Use with caution in patients with corneal or conjunctival virus infection, fungal infection and ocular tuberculosis. When combined with antibacterial drugs for bacterial infections, it should be used after the use of antibacterial drugs, and the drug should be stopped before the antibacterial drugs are stopped, so as not to mask symptoms and delay treatment, especially for those with active tuberculosis.

For patients who use this product for a long time, it is often necessary to increase the dose during surgery and within 3 to 4 days after surgery to prevent adrenal insufficiency. Intra-articular injection should not be performed in patients with acute suppurative arthritis. Prednisolone sodium phosphate is highly water-soluble and has a rapid effect, and can be used for intramuscular, intravenous and instillation; prednisolone acetate is a suspension that absorbs slowly and can be injected intramuscularly and intraarticularly.
